One Amendment Request

f you believe your report contains an error, you may request one amendment following its initial issue. Once an amended version has been issued, no further amendments can be made.
 

Minor errors can be corrected

We can correct typographical mistakes, spelling errors and administrative details where these do not change the clinical meaning or interpretation of the report.
 

How to Request an Amendment

Amendment requests should be submitted through our secure patient portal, clearly explaining the requested changes and why they are needed. Requests should normally be made within 28 days of receiving your report.
 

Protecting the accuracy of your report

To maintain accurate clinical records, changes to clinical opinion, diagnosis or outcomes cannot be accommodated. Every request is reviewed by the report author or an appropriately qualified clinician before a decision is made.
